Please refer to Forms page and download “payment form” or click here-Payment Form. After filling out form completely please email or fax form to our office. We recommend against emailing payment information as it’s less secure.

Another option is to contact us directly and pay over the phone. This will get the process started but we will still need the completed “payment form” prior to beginning any treatment.
